To help us understand how we can provide the best eye-care for your pet, we would like to ask if you could also provide us with some additional information and bring the following:
All the eye medications your pet is currently on.
A copy of any information you may have received from your veterinarian to do with your pet's eye problem(s). This can be recent information and anything you may have about any past eye problems.
The New Appointment Questionnaire filled. This can either be filled here, sent to us via email, fax, or you can bring it with you for your pet's appointment.
