We would like to explain how we would like to work in partnership with you and your client's to help diagnose and treat their pet's eye problems.
We believe that we work as part of a three-way partnership with you and your client's to help diagnose and treat their pet's eye conditions.
Our goal is to provide you and your client's with great service to ensure that your referral is an excellent experience for both of you.
We understand that good communication is of the utmost importance in maintaining a great relationship and providing the best possible care of your client's pets.
We will ensure that we provide you with up to date referral information the same day your client's pet is seem by us.
To help your client's understand about the eye problem's their pet has, we will undertake to spend time explaining this to them and providing understandable information in the discharges.
Whenever possible, we will involve you in performing any additional diagnostic testing such as blood-work and x-rays. We will forward pathology results to you. This will ensure that you have the most up to date information on your patient's health.
We welcome feedback (bouquets as well) on how we are providing you and your client's with specialist eye care services and look forward to working with you.
Please submit this form when you are referring your patient for a specialist ophthalmology exam. The more information you can provide us, the better we can understand your client's pet's eye health problems. We will contact your client directly once we receive your referral.
Appointment Requests:
We are currently receiving large numbers of new referrals from all over Ontario.
We are at our maximum daily capacity for one doctor and current staffing.
To maintain a safe level of patient care, we have to set a daily limit on how many patients can be scheduled or worked in.
NEW patients (And patients we have not examined in the past 2 years):
Our schedule is currently booked 4-5 months out for NEW patients. This is too long for many of the medical cases being referred.
Until we can catch up with the current backlog of referral patients, we are currently not accepting any new medical or surgical referral cases. We will revisit this in mid-March.
We will update the Clinic website when we are able to take on new referrals.

CURRENT Patients:
Patients that have been examined within the last year, will receive priority for appointments.
We will determine appointment “work-in” eligibility on a case-by-case basis.
